REGISTRATION FORMS For returning students and accepted new students. Students applying for admission should not complete these forms until after acceptance.

We are very excited to welcome our new and returning students to the Yeshiva next year. We look forward to a year of wonderful growth ahead!
FYHS has integrated the registration fee and yearly forms into our FACTS SIS system.
FYHS now requires that all families to set up and maintain an active FACTS financial account to process tuition or extracurricular school program fees. As such, during the registration process you will be prompted to login to your facts account or create an account if you do not already have one. Please complete a preliminary payment plan set up, so that when your tuition is determined your payment plan can begin in a timely fashion. You will not be charged for next year's tuition until a balance is put on your account. You will be asked to select a tuition payment option. This can be modified later or canceled if you pay in full.
A non-refundable registration and tuition deposit is required for each student.

- Tuition Deposit: $200 (Tuition deposit is applied against student's bill)
FACTS FAMILY PORTAL
To enroll in FYHS, you will need to access our FACTS Family Online Portal. If you do not have a login, please follow the Instructions for Creating a Family Portal Login listed below.
Instructions for Creating a FACTS Family Portal Login
- Please go to www.factsmgt.com
- Select Parent Log in from the menu bar and FACTS Family Portal from the drop-down menu.
- After the FACTS Family Portal Login screen opens, please select Create New Family Portal Account.
- Enter (FYHS-IL) into the District Code field.
- Enter in the Email field your email address as provided in your application to the school.
- Click the Create Account button.
- You will receive an email from FACTS SIS Customer Support containing a link that will allow you to create your username and password. For security purposes the link will remain active for 6 hours.
- Please click on the link. A Change/Create Password screen will open. You may use the default username provided, or create a new username. Then type in your desired password into the Password field and Confirm Field.
- Click on the Save Username and/or Password button.
- Close the window.
- Log into Family Portal as instructed above.
To access the Family Portal:
- Go to www.factsmgt.com
- Select Parent Log In from the menu bar and FACTS Family Portal from the drop-down menu.
- Type in your username and password. If you have forgotten your username or password, please click on the link provided.
- After logging in, click on Family in the left menu.
- Click on the Enrollment/Re-enrollment link.
The Online Enrollment system will open with a link to the enrollment packet. The process should take approximately 15 minutes to complete. Your information will be saved if you need to quit and come back later.
The Instructions and Enrollment Checklist page of the enrollment packet contains supplemental enrollment forms that also must be submitted. Further instructions on these forms are provided online.

TUITION & FINANCIAL ASSISTANCE FOR 2022-23
By policy and tradition students may apply for scholarship assistance where financial need can be demonstrated. Please submit your application through FACTS Grant & Aid Assessment. In addition, if you are a resident of Illinois and you will be requesting an internal FYHS Scholarship for next year, you must first apply for an Invest In Kids - STC scholarship through Empower Illinois and Big Shoulders Fund.
All applicants for such assistance will be reviewed by the Tuition and Scholarship Committee. Please visit www.htc.edu/facts to create your FACTS account.

INVEST IN KIDS
Fasman Yeshiva high School participates in the Invest In Kids - Illinois State Tax Credit Scholarship (STC) program, which has greatly reduced tuition costs and eased the financial burden for our families who qualify.
If you are applying for your other kids at other schools here in town, please follow the specific guidelines for each school.

FYHS FINANCIAL ASSISTANCE APPLICATION
Priority Deadline - May 15th
Scholarship applications must be submitted to FACTS Grant & Aid Assessment, along with complete Federal 2019 tax returns and W-2's.
When prompted on FACTS, please use the 2020-21 tuition numbers.
- 9th Grade In-Town: $25,205 (2020-21)
- 10th-12th Grades In-Town: $25,650 (2020-21)
- 9th-12th Dormitory: $33,770 (2020-21)
To set up a FACTS account and set up payment plans, please visit www.htc.edu/facts to create your account. If you are currently a FACTS user, you will be able to submit one application to multiple schools in the FACTS network.
Once an online application has been completed, the following information will need to be sent to FACTS to complete the application process:
Copies of your 2021 Federal tax forms including all supporting tax schedules and W-2 forms.
If you have not yet filed your 2021 taxes, please provide copies of your 2020 tax forms and 2020 W-2 forms until you file your 2020 taxes.
Copies of supporting documentation for Social Security Income, Welfare, Child Support, Food Stamps, Workers' Compensation, and TANF.
All supporting documentation can be uploaded in pdf format online. Documentation can also be faxed to 866-315-9264 or mailed to the address below.
Please be sure to include the applicant ID on all faxed or mailed correspondence.
